Location: Yoakum
Regency Integrated Health Services - - Responsibilities:
Implement the plan of care as established by the therapist and physician in a professional, competent manner;
Screen residents quarterly to identify declines in functional abilities and determine if therapeutic intervention is indicated;
Evaluate residents per MD order and establish treatment plans and goals;
Re-assess patients and caseload to determine need for continued services or discontinuation based on progress;
Provide discharge planning to maximize independence in the least restrictive environment
