Overview
Psychiatric Mental Health Nurse Practitioner (PMHNP) — Deer Oaks Behavioral Health — Long-Term Care Clinical Services. In this role, you will be assigned to a facility or facilities in your local community, building meaningful connections with the individuals you serve. You will conduct in-person psychiatric assessments, medication management, and implement Medicare-compliant gradual dose reduction strategies for psychotropic medications. We support geriatric-focused training and continuous professional development, providing accredited CEUs and ongoing case consultation with our experienced team of clinicians.
ClinicalCare
- Conduct comprehensive psychiatric evaluations and mental health assessments for LTC residents
- Develop, implement, and adjust individualized treatment plans
- Prescribe and manage psychotropic medications in accordance with Medicare and LTC best practices
- Implement and document Medicare-compliant gradual dose reduction (GDR) strategies
- Monitor and treat chronic mental health conditions including depression, anxiety, bipolar disorder, schizophrenia, and dementia-related behavioral disturbances
- Collaborate with interdisciplinary teams—nursing, medical providers, social services, rehabilitation, and facility leadership
- Complete accurate documentation of assessments, treatment plans, and progress notes using the electronic medical record (EMR) system
- Work closely with nursing, social services, rehabilitation, and medical providers to ensure coordinated psychiatric care
- Participate in care plan discussions, facility meetings, and case collaborations as needed
- Maintain complete and timely documentation using electronic medical records (EMR)
- Follow ethical, professional, and regulatory guidelines for psychiatric documentation and care delivery
- Build strong, professional relationships with facility staff and leadership
- Serve as a psychiatric resource and trusted partner for supporting residents’ mental and emotional needs
