Angels Care Home Health is seeking a licensed Physical Therapist to plan, implement, and evaluate patient care plans to restore or maintain well-being. You will evaluate patients, design PT treatment programs based on referrals, and coordinate with an interdisciplinary team to optimize outcomes.
Candidates must hold an active Physical Therapist license, have at least two years of experience, and maintain current CPR and Hepatitis profile. Knowledge of durable medical equipment is preferred.
