Join the Texas Health and Human Services Commission (HHSC) and help make a positive impact in the lives of Texans. This role supports the placement and transition of residents from state-supported living centers to community settings.

• Facilitate Community Living Discharge Planning (CLDP) process and develop transition plans.
• Serve as resource to interdisciplinary team members, community providers, and Local Authority staff.
• Assist with admissions, transfers, discharges, and related duties.
• Consult with facility professionals on support and service identification.
• Participate in planning and development of CLDP meetings, ensuring timely documentation.
• Review assessments and draft transition plans for distribution.
• Schedule and facilitate in‑services for providers prior to trial visits.
• Collaborate with disciplines to resolve assessment inconsistencies.
• Assist in policy development, administrative reports, studies, and specialized projects.
• Provide training to staff as needed.
• Perform related duties, including support for disaster response or COOP activation.

• Bachelor’s degree in a human services field (psychology, sociology, social work, special education, rehabilitation counseling, therapeutic studies, recreation).
• One year of experience working with individuals with intellectual disabilities.
• Preferred: experience in Intermediate Care Facilities for Individuals with Intellectual Disabilities and Home and Community‑based Services waiver programs.
• Knowledge of DADS community programs, including ICF/ waiver programs.
• Computer skills in Word, Excel, and other Microsoft Office products.
• Ability to handle multiple responsibilities and plan work activities.

• Effective oral and written communication.
• Professional working with individuals with intellectual disabilities, families, staff, and community providers.
• Ability to analyze facts and devise solutions.
