Bright Star Care - - Responsibilities:
Provide personal care services as needed to meet the client's needs;
Assist with daily tasks including feeding, meal preparation, linen changes, light housekeeping, and transportation to doctor appointments;
Assist client with mobility as defined in the plan of care, and document observed changes to the Director of Nursing;
Coordinate with RN oversight and care team to ensure plan adherence;
Maintain accurate care notes and care-logs using mobile time-tracking tools
