Description

This individual is responsible for individual and group therapy with participants in the PHP program. A candidate in this role models the values of Santé Center for Healing while working collaboratively within a Treatment Team and other agency departments to help patients achieve recovery goals. PHP therapists serve as a primary therapist for their clients and lead process groups, as well as provide traditional case management services, such as coordinating referrals, discharge planning, family communication, and correspondence with legal entities.

Requirements

Master’s Degree in a related field from an accredited university. Licensed for the state of Texas as an LPC, LPC-Associate, LCSW, LMSW, LMFT-Associate, LMFT, or equivalent, and has experience working with mental health and addiction populations. For persons in recovery, you must have a minimum of two years in recovery.

In order to complete the essential functions of this position the employee must be able to do the following with or without accommodations.

Must be able to:

· Work with adults with mental health and/or various addiction issues

· Distinguish the difference between appropriate and inappropriate behavior and professional boundaries

· Execute time management skills to handle multiple duties and priorities and stay current with deadlines.

· Communicate clearly verbally and in written format

· Apply motivational interviewing techniques when interacting with patients

· Complete documentation as outlined in Santé’s policies and procedures.

Summary

Responsibilities:

  • Manage caseload of patients to include clarification of the program content, program assignments, expectations of the program, goal setting for creating treatment plan, and discharge recommendations.
  • Meet with new patients to introduce self, complete new patient orientation, and explain treatment expectations. Conduct initial patient interview and psychosocial assessment within 72 hours of patient’s admission.
  • Provides timely documentation regarding all activities related to assessment, treatment and discharge planning, individual and group therapy, family and individual contact.
  • Develops treatment plans in conjunction with the patient and other members of the Treatment Team. Completes update of treatment plan and notes progress of patient on as per protocol. Explains treatment plans to the patient, including goals and expectations.
  • Provides individual and group therapy for assigned patients per program schedule.
  • Facilitates group sessions and conducts individual sessions with patient. Compiles case notes according to guidelines set by supervisor.
  • Participates in staffing and utilization review as scheduled by supervisor and other members of the Treatment Team. Makes recommendations to the Treatment Team about appropriate phase & level of the program.
  • Assigns patients appropriate therapeutic tasks to accomplish treatment goals. Provides feedback to patient regularly and as needed.
  • Communicates regularly with appropriate family members, legal personnel, referral sources, and other treatment entities and individuals.
  • Assists in compiling materials for use in group and individual therapy sessions.
  • Facilitates individual therapy sessions for Community Based Services (CBS) clients.
  • Provides coverage for other case managers or staff as needed.
  • Seeks clinical supervision and support when challenges arise.
