Location: McCamey

West Texas Centers is seeking a compassionate and motivated professional to join our team as an FEP Case Coordinator / MH Case Worker. The FEP Case Coordinator provides moderately complex case management services to individuals ages 15-30 who are experiencing their first episode of psychosis (FEP). This position carries a limited caseload of approximately 20 individuals and serves clients across all 23 counties in the West Texas Centers service area.

Travel to other counties may be required based on client needs. The position will work approximately 50% as an FEP Case Coordinator and 50% as an MH Case Worker.

Salary: $21.84 per hour

  • Build positive, supportive relationships with participants and their families.
  • Provide ongoing case management and support to individuals experiencing their first episode of psychosis.
  • Participate as an active member of the treatment team and collaborate with clinical staff and other service providers.
  • Develop and maintain relationships with clients, families, and community providers.
  • Coordinate medical, social, educational, employment, substance use, and other needed services.
  • Assist participants in developing skills for illness self-management and community living.
  • Use a shared decision-making approach to develop and modify individualized recovery plans.
  • Identify when participants may benefit from Cognitive Behavioral Therapy and coordinate with the LPHA as appropriate.
  • Provide specialized training to staff as requested.
  • Maintain accurate and timely documentation.
  • Work flexible hours to meet the needs of clients and families.
MH Case Worker Responsibilities
  • Serve as the primary case manager for an assigned caseload.
  • Assist individuals in accessing medical, social, vocational, educational, and other needed services.
  • Develop and monitor individualized service plans.
  • Coordinate services and make appropriate referrals.
  • Provide advocacy, crisis resolution, and liaison services.
  • Provide rehabilitative services designed to improve functioning and independence in the community.
  • Provide symptom management and wellness support.
  • Provide psychosocial rehabilitation and skills training.
  • Maintain frequent contact with individuals served, families, and other service providers.
  • Complete required documentation and paperwork accurately and timely.
  • Work independently while maintaining communication with the treatment team.
Required:

Bachelor's degree from an accredited college or university with a major in a social, behavioral, or human services field.

Knowledge of mental health and the DSM-5.

Ability and willingness to travel throughout the West Texas Centers service area.

Ability to work flexible hours based on client and family needs.
