Help us help others:

A Mental Health Care Coordinator will assess needs,develop treatment plans, and effectively collaborate with individuals and their family members. The Care Coordinator will assess strengths and needsand, in collaboration with the individual, develop a person-centered recoveryplan. The Care Coordinator will provide skills training, crisis prevention/management, linkage, and referral to support goal progress for children/adults and families living with mental health challenges. Thiscompassionate, caring, and supportive team member will contribute to the skillsimprovement, achievements, and overall potential success of a child/adult and their state of well-being.

You will get to:

• Assess individual needs and collaborate with them to develop and execute effective recovery and lifemanagement plans.
• Provide relevant skills training to improve daily life.
• Collaborate with service providers and agencies to be a liaison for effective care and positive outcomes.
• Write and document articulatelywhile using correct grammar so that documentation is clear and valuable.
• Maintain compliance with alllegal requirements and company policies and procedures.
• Consistently meet with individuals to maintain progress as well as compliance.

You'll definitely need:

• Bachelor's degree from an accredited college or university in social, behavioral, or human services with the ability to be credentialed as a Qualified Mental Health Professional (QMHP-CS)
• Maintain a valid Texas state Driver's License and auto insurance.
• To complete and maintain both CPR certification and Prevention and Management of Aggressive Behavior training within the first 3 months of employment

Preferred -

• At least one year of experience implementing the Child and Adolescent Needs and Strengths Assessment or the Texas Resilience and Recovery Model for Adults or Children.

• Be available to manage crisis situations as they occur in sometimes off-scheduled hours (this is typically an infrequent occurrence)
• Employ excellent verbal and written communication skills to engage and report effectively.
• Maintain a well-organizedschedule while allowing room for flexibility and adjustments.
• Maintain accurate and timelycollaborative documentation.
• Sustain a calm demeanor whenunder pressure to meet deadlines and handle sometimes difficult situations.
• Provide exemplary customer service and care to individuals with their best interests at heart.
• Work autonomously to complete assignments within company and state parameters with a high level ofquality.
• Must become certified in PMAB, First Aid, and CPR within three months of employment and maintain certification without interruption thereafter.
