Hill Country Mental Health and Developmental Disabilities Centers is seeking a Care Coordinator for Blanco County to assess needs, develop person-centered recovery plans, and collaborate with individuals and families.
You will provide skills training, crisis prevention/management, and referrals, document clearly, and maintain compliance with policies. A Bachelor's in social/behavioral/human services and a Texas driver’s license are required; CPR/PMAB training within 3 months is expected.
