Occupational Therapist; OT

We are seeking a Home Health Occupational Therapist to deliver patient-centered care in a home setting. In this role, you will

• evaluate patients' functional abilities, assess home safety, and develop personalized treatment plans to improve independence and daily functioning
• provide therapeutic interventions, including retraining in daily activities, fabricating splints, and training on adaptive devices
• support patients returning to work through functional assessments and educate patients, families, and caregivers on care plans and safety
• collaborate with interdisciplinary teams, document care accurately within EMR, and participate in case conferences
• supervise Certified Occupational Therapy Assistants and oversee home health aide care when therapy is the sole skilled service

Ideal candidates will demonstrate strong clinical judgment, excellent communication, organization, and a compassionate, proactive approach.

Qualifications include graduation from an accredited OT program, current NBCOT registration, state licensure, CPR certification, and a valid driver’s license with reliable transportation.

Preferred experience includes at least one year of clinical work, especially in home health.
