Work Hours:
- PRN / Part-Time — Hours vary based on client assignments and current care needs. Additional hours may become available as new clients begin services.
Provides personal care services under the direction of the Registered Nurse or Therapist. The Aide is assigned to specific clients by the Registered Nurse or other appropriate professional who performs services for clients as necessary to maintain their personal comfort and well-being.

Provides Supervision To:NA – No supervision responsibilities
Requirements:- (+1 year of Caregiving Experience)
- Active CPR Certification
- Recent TB Test (12 month two-step, or 2 year blood test)
- Provide companionship and, if authorized, transportation to clients according to the care plan.
- Assist client with general Activities of Daily Living (ADLs) such as bathing, meals, and safe ambulation/transfers.
- Report any changes in the client’s condition to the supervisor as outlined in the care plan.
- Document care provided and maintain accurate records for the Agency.
- Work collaboratively with the client’s care team and ensure effective communication.
- Promote client safety by following infection control practices and reporting unsafe conditions.
- Attend in-service programs to meet compliance and notify supervisors of any educational needs.
- Meets one (1) of the following:
- A minimum of one (1) year of full-time experience in direct client care in an institutional setting.
- One (1) year of full-time experience within the last five )5 years in direct client care in an agency setting.
- Satisfactory completion of a training and competency evaluation program that complies with Texas regulation 558.701 plus experience providing home health services in the past 24 months.
- Submission of documentation to the Agency from the director of programs or the dean of a school of nursing verifying that the individual is a nursing student who has demonstrated competency in providing basic nursing skills in accordance with the school's curriculum.
- Is listed on the Texas Health and Human Services Commission Nurse Aide Registry and in good standing.
- Be at least eighteen (18) years of age.
- Demonstrated ability to read, write, and follow a written care plan.
- Good verbal, written, and interpersonal skills.
- Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair.
- Current CPR certification.
- Recent TB test (12 months)
- (1+ year of caregiving experience)
