Crossover Health is seeking a Health Coach to support members with weight management, stress management, nutrition, physical activity and other healthy lifestyle changes within our primary care model.
You will deliver evidence-based coaching programs, collaborate with care teams to assess, plan, implement and evaluate action plans, and lead onsite wellness sessions and classes. A Health Coaching certificate and 3+ years in outpatient care, plus BLS, are required.
