You will coordinate crisis intervention, discharge planning, referrals, and bereavement support as part of the case management team.
Requirements include a Master of Social Work with TX licensure (LCSW/LMSW/LMSW-AP) and at least 1 year of experience. This full-time position includes benefits and PTO from Day 1.
