This position pays a base range of $70,500 to $96,900 per year.

• Provide admission, case management, and follow‑up skilled nursing visits for home health patients.
• Administer ongoing care and case management for each patient, providing necessary follow‑up as directed by the Clinical Manager.
• Collaborate with physicians to develop and revise initial treatment plans based on physician orders and patient assessment.
• Coordinate care with physical therapists, occupational therapists, home health aides and external providers.
• Communicate patient care, condition, and progress to physicians and Clinical Manager on a continuous basis.
• Assist patients and families to achieve optimal resolutions of needs and problems through care plan implementation.
• Supervise home health aides, certified home health aides and personal care workers and provide written personal care instructions.
• Discharge patients after consultation with the physician and Clinical Manager, preparing and completing necessary clinical documentation.
• Prepare and update medical documentation, including case conferences, medication order changes, re‑certifications, progress and care plan changes.

• Diploma, Associate or Bachelor degree in nursing.
• Minimum of one year of nursing experience preferred; experience in emergency, ICU, acute care or home health a plus.
• Current and unrestricted Registered Nurse licensure.
• Current CPR certification.
• Strong organizational and communication skills.
• Valid driver’s license, auto insurance and reliable transportation.

• Pay per visit/unit: $45.00–$63.00.
• Base pay: $70,500 – $96,900 per year.
