Licensed Practical Nurse

Overview

The LPN/LVN provides professional nursing care to patients within an assigned unit, clinic or region. Responsibilities include assessing patient health problems and needs, taking vital signs, administering medications, developing and implementing nursing care plans, and maintaining medical records. May advise patients on health maintenance and disease prevention or provide case management.

Location:

Conviva Goliad, 1567 Goliad Rd, San Antonio, TX 78223. Reports to the Manager, Clinic Administrator.

• Provide professional nursing care to patients within an assigned unit, clinic or region.
• Assess patient health problems and needs; take vital signs and administer medications.
• Develop and implement nursing care plans; maintain medical records.
• Advise patients on health maintenance and disease prevention or provide case management as needed.

• Current licensure as an LPN or LVN.
• Proficiency in administrative duties, as the role is highly administrative in nature.

• Bilingual in English/Spanish.
• Phlebotomy experience.
• Experience with Electronic Medical Records (EMR).
• Experience with HEDIS.

• This role is considered patient facing and is part of Humana s Tuberculosis (TB) screening program. If selected, you will be screened for TB.

• Pay Range: $48,900 - $66,200 per year (base pay for full-time, may vary by location and experience)
• Bonus eligibility based on company and/or individual performance.
