Position Summary
The Licensed Vocational Nurse (LVN) provides basic nursing care under the clinical supervision of a Registered Nurse (RN) to individuals with developmental and intellectual disabilities (IDD). The LVN assists with routine health monitoring, medication administration, and support for daily living activities, ensuring services are delivered in accordance with each individual's care plan, nursing standards, and Caregiver policies. This role requires attention to detail, compassion, and a commitment to high‑quality, person‑centered care.
EssentialFunctions
- Uphold principles of dignity, respect, and person‑centered care in all interactions with individuals.
- Provide routine nursing care and health monitoring as directed by clinical supervisor.
- Administer medications and treatments as directed by clinical supervisor.
- Observe and report changes in client health status or behavior to clinical supervisor.
- Assist with activities of daily living (ADLs) including hygiene, mobility, nutrition, and safety.
- Reinforce training for direct support staff regarding basic health and infection control.
- Participate in the implementation of individualized health care and behavior support plans.
- Participate in the provision of healthcare services onsite by other healthcare providers via telehealth or in‑person visits.
- Ensure proper storage, handling, preparation, and labeling of all medications.
- Maintain accurate medication inventory and documentation, and immediately report any discrepancies.
- Maintain accurate, timely, and confidential clinical documentation in compliance with Caregiver policies and HIPAA requirements.
- Ensure a safe environment for individuals, staff, and others and immediately report safety concerns.
- Respond immediately to emergencies and provide nursing assistance.
- Participate in nursing and interdisciplinary team meetings as directed.
- Provide nursing support for assigned on‑call shifts, including after‑hours as needed.
- Graduate of an accredited LPN/LNV program.
- Minimum of 1 year of clinical nursing experience preferred.
- Minimum of 1 year of experience with IDD/DD populations or LTC preferred.
- Active LPN/LVN license from state of residence. If work location is in a different state, must hold a multi‑state LPN/LVN license in accordance with the rules for Nurse Licensure Compact (NLC).
- Valid driver’s license, with proof of auto insurance coverage.
- Basic Life Support (BLS) certification.
- Understanding of developmental and intellectual disabilities and their implications for health and daily living.
- Knowledge of basic nursing care principles, medication administration protocols, and infection control practices.
- Ability to observe and document clinical findings accurately.
- Ability to adapt care strategies to the individual needs and preferences of each client.
- Ability to communicate effectively with clients, families, interdisciplinary team members, and supervisors, verbally and in writing.
- Experienced in using electronic health records and documentation systems.
- Ability to work independently while recognizing when to seek guidance or elevate concerns.
- Ability to manage multiple priorities while maintaining a calm, supportive environment.
- Proficiency or ability to learn electronic health record (EHR) program and Microsoft 365 programs including Outlook, Teams, and Word.
