Position Overview
Join a company that prioritizes quality home‑care. Licensed Practical/Vocational Nurse (LPN/LVN) position in New Braunfels, TX, working 2‑4 shifts per week (6:00 a.m.‑6:00 p.m.). Provide skilled nursing under the supervision of a Registered Nurse for patients, including those who require feeding tubes and medication administration. Maintain confidentiality, professional boundaries, and comply with Aveanna policies and state/federal regulations.
Essential Job Functions- Deliver and coordinate quality patient care in compliance with physician orders.
- Continuously observe, assess, and document patient condition and care needs.
- Report changes in patient condition to the supervisor and/or physician promptly.
- Participate in, implement, and update the nursing care plan.
- Take appropriate nursing actions based on assessment to achieve expected outcomes.
- Recognize changes in patient needs to prevent or reduce risk.
- Provide care utilizing infection control measures that protect staff and patients.
- Educate patients and families regarding disease process, self‑care techniques, and prevention strategies.
- Maintain knowledge of nursing competencies through continuing education and internal learning programs.
- Graduate of an accredited nursing school.
- Maintain an active, unrestricted LPN/LVN license in the state of practice.
- Current CPR certification (hands‑on component).
- Demonstrated proficiency in clinical assessments, nursing documentation, and compliance with care plans and policies.
- Current TB skin test within the last 12 months.
- Reliable transportation.
- Additional state‑specific requirements:
• South Carolina – 1 year of pediatrics experience;
• California – 1 year of experience under a current nursing license;
• Louisiana – 1 year of experience as a licensed nurse; continuing education as required by state.
- Six months or more recent experience as a Licensed Nurse in a clinical care setting.
- Home‑health experience.
- Attention to detail.
- Time management.
- Effective problem‑solving and conflict resolution.
- Strong organization and communication skills.
- Speak, read, write, and understand English.
- Travel as required, including rural areas.
- Lift up to 50 pounds and reposition patients without assistance.
- Walk, stand, bend, kneel, reach, twist, and climb stairs for prolonged periods.
- Good vision, hearing, sense of smell, and touch.
- Respond physically and mentally to emergency situations in the home or during transport.
- Work in a wide variety of settings, which may involve exposure to allergens and bodily fluids.
- Possible exposure to blood and infectious diseases.
Additional duties may be assigned based on patient needs or organizational change. As an employee handling Medicare and Medicaid funds, comply with all health‑related requirements and vaccinations as mandated by jurisdiction.
