Overview
We are seeking an experienced Licensed Practical Nurse (LPN) – Home Health for a 13-week contract assignment in Temple, Texas. The ideal candidate will have home health experience, strong patient assessment skills, and proficiency in OASIS documentation. This role involves providing skilled nursing care in patients' homes, completing admissions, resumptions of care, recertifications, and discharge visits, while ensuring accurate and timely documentation.
The successful candidate will collaborate with physicians, caregivers, and interdisciplinary teams to deliver high-quality patient-centered care, manage a productive caseload, and support positive patient outcomes throughout the continuum of care.

Shift: Days
Pay: $40-45/hr
Contract Duration: 13 Weeks
Key Responsibilities- Provide skilled nursing care to patients in their homes according to physician orders and individualized care plans.
- Perform patient assessments, medication management, wound care, and disease management education.
- Complete OASIS documentation, admissions, recertifications, resumptions of care, and discharge assessments.
- Monitor patient progress and communicate changes in condition to the interdisciplinary care team.
- Educate patients and caregivers on treatment plans, medications, and disease management.
- Maintain accurate and timely documentation in compliance with agency and regulatory requirements.
- Coordinate care with physicians, therapists, case managers, and other healthcare professionals.
- Active LPN License in Texas.
- Previous Home Health experience required.
- Home Care Home Base (HCHB) experience preferred.
- Candidates without HCHB experience may be considered if willing to complete online training before start.
- Strong knowledge of OASIS documentation and home health regulations.
- Valid driver's license, reliable transportation, and ability to travel throughout the coverage area.
