Qualifications
  • Current LVN/LPN license in practicing state.
  • Current BLS/CPR certification or willingness to acquire one.
  • Experience in nursing management is preferred but not required.
Responsibilities
  • Follows established procedures for charting and incident reporting.
  • Experience in treating wounds/woundcare.
  • Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
  • Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
  • Performs resident skin assessments, obtains treatment orders, administers treatment as ordered, and documents appropriately to ensure that resident skin integrity is improved and/or maintained.
  • Documents findings appropriately and contacts physician to make recommendations for new or revised treatment orders; monitors the progress of wound healing and makes recommendations for changes as appropriate.
  • Maintains records of pressure ulcers and skin alterations including risk assessment, prevention and minimizing activities; performs and records weekly pressure ulcer assessment and measurement documentation; identifies and maintains effective methods to gather pressure ulcer and skin alteration related information; calculates weekly and monthly pressure ulcer rates.
  • Investigates and identifies center practices and risk minimizing measures, including accurate completion of Braden risk scales on admission, implementation of preventive interventions.
  • Monitors timely implementation of preventive interventions for at risk residents, including new admissions, individuals with skin alterations and individuals at risk for development of skin alterations.
  • Educates newly hired nursing staff on Skin Practice Guide requirements and Skin Resource Library including wound assessments, preventive interventions, care planning, treatment selection options, documentation requirements and education resources available.
  • Evaluates availability of needed supplies for wound care and coordinates ordering with central supply.
  • Assists the Director of Nursing and medical director with case review for center acquired pressure ulcers.
  • Monitors changes in state and federal regulations and current industry-standard practices; keeps current with industry trends through ongoing specialized education in the area of skin and wound management.
  • Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
  • Documents and notifies applicable departments of any patient’s room changes, LOA, new admits, or discharges.
  • Makes rounds with physicians when necessary.
