Licensed Practical / Vocational Nurse – LPN/LVN

Join a local office that prioritizes compassionate care for a special client/patient. We need a team of LPNs/LVNs to provide skilled nursing under the supervision of a Registered Nurse.

Position Overview

The LPN/LVN provides and documents skilled nursing care, following the care plan and physician orders, and maintains confidentiality and professional boundaries at all times.

Essential Job Functions
  • Deliver and coordinate quality patient care in compliance with physician orders.
  • Continuously observe and assess patient condition and report changes to the supervisor or physician.
  • Document all activities, assessments, nursing actions, responses and coordination of care promptly.
  • Participate in, implement, and update nursing care plans.
  • Take appropriate nursing action based on assessment to achieve expected outcomes.
  • Recognize changes in patient needs and implement interventions to prevent or reduce risk.
  • Comply with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act.
  • Provide care using infection control measures that protect staff and patients according to OSHA standards.
  • Educate patients and families regarding disease processes, self‑care techniques, and prevention strategies.
  • Maintain knowledge of nursing competencies through education and skill evaluation.
Requirements
  • Graduate of an accredited nursing school.
  • Current and unrestricted state license as an LPN/LVN.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care policies.
Additional State‑Specific Requirements
  • South Carolina – 1 year of pediatrics experience.
  • California – 1 year of experience working under a current nursing license.
  • Louisiana – 1 year of experience working as a licensed nurse.
  • Continuing education as required by state.
Preferences
  • 6 months of recent experience as a licensed nurse in a clinical setting.
  • Home health experience.

Physical Requirements
  • Speak, write, read and understand English.
  • Travel and lift up to 50 pounds.
  • Reposition patients and move equipment without assistance.
  • Prolonged walking, standing, bending, kneeling, reaching, twisting, and climbing stairs.
  • Visual and hearing acuity; strong sense of smell and touch.
  • Respond physically and mentally to emergency situations in the home or during transport.
Environment
  • Function in varied environments with potential exposure to allergens.
  • Possible exposure to blood, bodily fluids and infectious diseases.
