Professional Nurse Practitioner (Skilled Nursing) – Avir Health Group
Avir Health Group is searching for a qualified Nurse Practitioner to join our team in a Skilled Nursing Facility (SNF). In this role you will serve as a clinical leader, working collaboratively with the care team to oversee both short‑ and long‑term residents.
Responsibilities include admissions, discharges, monthly stability checks, evaluations for changes in condition, advanced care planning, comprehensive assessments, and prescribing treatments.
