Location: Lefors
Matrix Medical Network is seeking qualified Nurse Practitioners to conduct in-home Comprehensive Health Assessments (CHA) for adults and geriatric members across the United States. Each visit lasts up to one hour and focuses on medical history, adherence, and social factors to identify care gaps and improve overall health.
Providers must hold state NP licensure with prescriptive authority and board certification in Adult, Family, or Gerontology specialties.
