Job Description: Assess and evaluate residents’ needs for medical, psychosocial and personal care. Initiates and implements all phases of the nursing process. Delivers and documents resident care. OSHA Bloodborne Pathogens Class I.

JOB QUALIFICATIONS
  • Education:

    Graduate of an accredited School of Vocational or Professional Nursing.
  • Personal

    Job-Related Skills:

    Effective oral and written communication skills. Management and leadership abilities. Knowledge and experience of Texas Standards for Nursing Homes preferred.
  • Licensure, Registry or Certification:
    Licensed by the State of Texas as a Vocational or Registered Nurse.
EXPERIENCE
  • Prior

    Work Experience:

    One year experience in a medical setting preferred.
  • Technical Training:
    Current CPR.
  • Physical and Mental Requirements:
    Duties of the position require extended periods of standing and walking. Bending, stooping, twisting, reaching, and lifting up to 50 pounds. Occasional heavy lifting when moving or supporting patients who may suddenly become weak or helpless.
DUTIES AND RESPONSIBILITIES
  • Completes admission assessment of the new resident within three hours of admission.
  • Prioritizes assigned resident’s needs and plans the care.
  • Administers prescribed medications per nursing direction.
  • Orders and receives medications from pharmacy as needed.
  • Executes physician orders under the direction of the Director of Nursing.
  • Provides resident care as indicated in the care plan according to established standards for care plan review.
  • Documents observation on the daily assessment sheet.
  • Charts nursing notes reflecting problems identified on the care plan.
  • Documents on special sheets as per facility policy.
  • Reviews and updates the nursing care under the supervision of the Director of Nursing, indicating problems and resolutions.
  • Initiates resident/family teaching when advised.
  • Makes rounds and assesses care needs for residents.
  • Plans and prioritizes care for an assigned group of residents with the supervision and direction of the Director of Nursing.
  • Reports all changes in residents’ condition and discusses the pertinent data with the Director of Nursing.
  • Notifies other departments of resident needs.
  • Ensures that all assigned residents receive their treatments, laboratory work, etc. on a specified basis.
  • Provides information and assistance according to resident/family learning needs.
ADDITIONAL RESPONSIBILITIES
  • Maintains residents' dignity and rights at all times.
  • Removes from residents’ rooms and reports any equipment that is not in working order.
  • Limits supplies and equipment that is in residents’ rooms.
  • Identifies potential resident safety problems and implements precaution according to policy.
  • Implements infection control and safety procedures.
  • Checks equipment such as suction machines, oxygen tanks for level content and other emergency equipment.
  • Assists with narcotic check for control.
  • Directs care given in emergencies until a physician can be reached or arrives.
  • Reports changes in response of residents to physicians.
  • Arranges for transfer of residents as needed.
  • Completes incident reports for residents and staff members promptly following incidents.
OTHER SIGNIFICANT REQUIREMENTS
  • Maintains confidentiality while working with confidential matters on a daily basis.
  • Provides service direct to residents while maintaining a positive patient relations atmosphere.
  • Follows aseptic procedures and precautions when involved in direct patient contact or handling potentially infectious materials.
  • Performs additional support duties when the Administrator, Director of Nursing or other designated staff person is absent.
