Job Summary:
We are seeking a highly skilled and experienced Home Health - RN Case Manager to join our team. The ideal candidate will have a strong background in home health care and be able to provide exceptional care to our patients.
Responsibilities:- Performs skilled nursing services under the supervision of a staff DON, ADON.
- Observes, records, and reports to the supervisor on the general physical and mental conditions of the patient.
- Prepares clinical and or progress notes and incorporates them into the clinical record at least weekly.
- Reports to the patient's physician and other appropriate personnel pertinent changes in the patient's condition.
- Receives and follows physician orders within the realm of practice for an LVN and within the standards of home health practice but may not alter the POC. (LVN's ONLY)
- Participates in an in-service program, attending at least 2 in-services/educational meetings per year.
- Check and approve all interim orders.
- Work the "Hold List" and initial off on each patient when complete.
- Triage phone calls and all MD calls.
- Monday-follow up with your patients from weekend after receiving reports.
- Coordination/Follow up.
- Follow admits on your team and plotting of therapy, front loading visits, wound evals, sup visits, etc.
- Review 485/Lock OASIS in proper time.
- Assist with referrals.
- Scheduling.
- Check email frequently throughout the day and respond promptly.
- Oversee team compliance/accountability.
- Educate your team members as needed.
- Work on your Case Conference throughout the week.
- Participates in staff case conference weekly regarding patient care.
- Insurance Authorization.
- Current RN license in the state of Texas
- Minimum of 1 years of experience in home health care as RN or LVN preferred.
- Strong clinical skills and knowledge of disease management
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong organizational and time management skills
- Experience with electronic medical records
- Valid driver's license and reliable transportation
