Nbutexas is seeking an experienced RN Case Manager to coordinate patient care from admission to discharge and ensure safe, efficient transitions from hospital to home. You will serve as the link between patients, families, and care teams to optimize outcomes.
You will analyze utilization, participate in rounds, and advocate for patients while meeting CMS and payer guidelines. A valid RN license and 2+ years' clinical experience are required; CCM/ACM certification is a plus.
