LVN Charge Nurse

Touchstone Communities is hiring a PRN LVN Charge Nurse to support our skilled nursing and rehabilitation residents across our Texas communities. In this field-based, on-site role, you will provide hands-on clinical care, administer medications, and oversee nursing staff on assigned shifts. Working within a faith-based, servant-leadership culture, you will champion compassionate, person-centered care, support individualized care plans, and collaborate with therapy and interdisciplinary teams to enhance residents' recovery, comfort, and dignity while ensuring regulatory compliance and high-quality clinical outcomes.

Responsibilities

• Provide direct nursing care to residents in a skilled nursing and rehabilitation setting
• Serve as Charge Nurse on PRN shifts, supervising LVNs, CNAs, and support staff
• Administer medications and treatments per physician orders and facility protocols
• Monitor resident conditions, document changes, and communicate with interdisciplinary team and families
• Support care planning, assessments, and discharge planning to promote optimal outcomes
• Ensure compliance with state, federal, and facility regulations and infection control standards
• Educate residents and families on care plans, therapies, and health management
• Promote a resident-centered, faith-based, compassionate care environment
