LVN Charge Nurse

Touchstone Communities is seeking a compassionate LVN Charge Nurse for day shifts in our skilled nursing and rehabilitation community. In this role, you'll coordinate and deliver resident-centered clinical care, administer medications and treatments, oversee nursing assistants, and ensure accurate documentation in accordance with state and federal regulations. You'll collaborate with an interdisciplinary team, support care plans, communicate closely with residents and families, and model our faith-based, servant-leadership culture while promoting safety, dignity, and quality outcomes.

Responsibilities

• Provide direct nursing care to residents on the day shift, including assessments, treatments, and monitoring of conditions.
• Administer medications and therapies accurately and safely in accordance with physician orders and facility protocols.
• Supervise and support CNAs and other support staff, delegating tasks and ensuring quality, timely care.
• Participate in developing, updating, and implementing individualized care plans with the interdisciplinary team.
• Maintain thorough, timely clinical documentation in the EHR and paper records as required by policy and regulation.
• Communicate effectively with residents, families, and healthcare providers regarding care, changes in condition, and goals.
• Ensure compliance with state and federal long-term care regulations, infection control, and patient safety standards.
• Support and model Touchstone's servant-leadership, faith-based, and person-centered care culture in daily practice.
