In Home Healthcare LVN

Aveanna Healthcare is seeking a compassionate In Home Healthcare LVN for part-time weekend day shifts. In this role, you'll provide skilled nursing care to pediatric and/or adult patients in their homes, including assessments, medication administration, wound and respiratory care, and monitoring of chronic conditions. You will follow individualized care plans, document care accurately, and communicate closely with RNs, physicians, and families.

Responsibilities

• Provide skilled nursing care to pediatric and/or adult patients in their homes on weekend day shifts
• Perform patient assessments, monitor vital signs, and report changes in condition
• Administer medications and treatments per physician orders and care plans
• Deliver wound, respiratory, tracheostomy, and G-tube care as required
• Document all care accurately in accordance with clinical and regulatory standards
• Collaborate with RNs, physicians, and care teams to update individualized care plans
• Educate patients and family members on disease management and home care techniques
• Follow infection control, safety protocols, and emergency procedures
• Maintain professional communication and demonstrate compassion and respect in all interactions
