About House Calls

House Calls is a unique, outcomes-driven program that delivers annual in-home health assessments to Medicare Advantage and other health plan members. As an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you'll focus on preventive care, patient education, and collaboration with primary care providers to improve overall health and wellness.

This is a non-prescribing, field-based role that allows you to spend meaningful time with patients while enjoying the autonomy and flexibility that comes with a mobile care model.

Primary Responsibilities
  • Conduct comprehensive in-home health assessments, including:
    • Medical history review
    • Medication reconciliation
    • Vital signs and physical examinations
    • Evidence-based screenings and point-of-care testing, as appropriate
  • Identify and document diagnoses to support care management and future treatment planning
  • Partner with primary care providers by communicating findings and helping address gaps in care
  • Recognize urgent or emergent situations and take appropriate action
  • Educate members on disease management, medications, and treatment adherence
  • Assess social determinants of health and connect members with community resources when appropriate
