Licensed Vocational Nurse, Home Health

Accent Care seeks a Licensed Vocational Nurse for home health services supporting Hanford Pharmaceuticals employees and families. The LVN will deliver in-home skilled nursing care, administer medications, monitor chronic conditions, and coordinate with physicians and interdisciplinary teams. Responsibilities

• Provide in-home skilled nursing care and treatments to assigned patients
• Administer medications and therapies per physician orders and company protocols
• Monitor and document patient status, vital signs, and responses in EHR systems
• Develop and update individualized care plans in collaboration with RNs and physicians
• Educate patients and families on medications, disease management, and home care routines
• Ensure strict adherence to safety, infection-control, and regulatory standards
• Coordinate care and communicate changes with interdisciplinary healthcare teams
• Participate in case conferences and quality improvement initiatives
• Support safe transitions of care from hospital or facility to home
• Maintain accurate, timely clinical documentation and reports
