Matrix Medical Network seeks nurse practitioners to perform comprehensive health assessments in members’ homes or skilled nursing facilities, reviewing medical history, medications and social factors to identify care gaps. Each visit lasts up to one hour to gather insights beyond typical visits.
Providers assess and educate, administer point-of-care tests when needed, and coordinate with PCPs, case managers and families to improve outcomes.
