Job Description:
Case Manager III - Female Cognitive Pre-Release Program – Marlin Unit (028890)
Provides highly complex case management work. Work involves reviewing and tracking case management activities; assisting in developing program goals, objectives, and procedures. Works under limited supervision with significant latitude for independent judgment.
Essential Functions- Provides ongoing case management and reviews and tracks case management activities for inmates; interviews inmates to gather information to assess service needs and review progress and compliance within the program; participates in team meetings for individualized program planning and reentry planning.
- Provides resource information for reentry planning; assists in development of program schedule.
- Provides liaison with service providers; assists with intra‑agency and inter‑agency services coordination and referrals.
- Documents and maintains inmate program activity and case management records; prepares and reviews reports.
- Prepares and presents educational materials in individual and group settings.
- Performs a variety of marginal duties not listed, as determined and assigned as needed.
A. Education, Experience, and Training
- Knowledge of principles, methods, and practices of case management.
- Knowledge of group dynamics.
- Knowledge of agency and departmental organizational structure, policies, procedures, rules, and regulations preferred.
- Skill to communicate ideas and instructions clearly and concisely.
- Skill to coordinate with other staff, departments, officials, agencies, organizations, and the public.
- Skill to develop, implement, and monitor service or treatment plans.
- Skill to interpret and apply rules, regulations, policies, and procedures.
- Skill in problem‑solving techniques.
- Skill to prepare and maintain accurate records, files, and reports.
- Skill to review technical data and prepare technical reports.
