** who assists in addressing the social, emotional, and practical needs of residents in long-term care facilities under the supervision of a licensed social worker.
** Role Overview
** A Social Service Designee, also known as a Social Service Aissstant, is an entry-level professional who supports the social services department in long-term care facilities. They work under the guidance of licensed social workers or administrators to ensure residents’ social, emotional, and medically-related needs are met.
** Key Responsibilities**
* ** Resident Advocacy:
** Act as a liaison for residents, representing their social interests and ensuring their preferences and concerns are communicated to the care team.
* ** Care Planning and Coordination:
** Assist in developing, updating, and implementing residents’ care plans, collaborating with nursing staff, therapists, and other healthcare professionals to address social and emotional needs.
* ** Family Communication:
** Maintain open communication with residents’ families, providing updates and guidance while supporting them through transitions.
* ** Resource Management:
** Inform residents and families about community resources, financial assistance programs, and support services, ensuring appropriate referrals.
