Location: Canadian
CMHA Algoma is seeking a Housing Case Manager to deliver community-based case management and housing support to individuals facing mental health, addiction, or intellectual disability challenges. You will assess needs, plan for crises, and connect clients with resources to improve wellbeing, in alignment with CMHA Algoma policies and ethics.
You will work both independently and with a team, meeting clients in their environments and collaborating across housing rent supplement initiatives.
