Under the supervision of the Specialized Health and Social Services (SHSS) Program Manager , performs complex and comprehensive public health social work to clients requesting services through the Medicaid benefit of Personal Care Services (PCS) and/or the Community First Choice (CFC) Program. Collaborating with medical providers, community partners, and other state agencies to address needs. Maintaining current and accurate documentation of all contacts/case activities in case files and designated databases according to program policies/applicable state and federal laws.
Educates the public and providers about program services. Works under general supervision, with moderate latitude for the use of initiative and independent judgment.
Serves as the Children and Pregnant Women (CPW) program Liaison for Region 7, providing consultation, technical assistance, and training to prospective and current contracted CPW case management providers on the requirements and reporting expectations for the CPW program. Routinely works with HHSC Case Management to fulfill CPW program requirements. Conducts courtesy reviews for CPW program providers and quality assurance reviews for SHSS program social workers.
Coordinates quality assurance management to ensure the implementation of program policies and procedures and monitor compliance for CPW and Personal Care Services (PCS)/Commumity First Choice (CFC) programs effectiveness and quality. Assists in training new hires to SHSS program.
Conducts outreach and actively collaborates with community partners, by participating in various community organizations such as the (CRCG) Community Resource and Coordination Groups., Interagency groups as well as local coalitions. Educates the public and CPW providers, about available resources and program services in order to improve the health and well being of the community.
Other duties as assigned, which includes actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation.
Essential Job Functions (EJFs):
- Liaison to the Children and Pregnant Women (CPW) Program Under the supervision of SHSS Program Manager and SHSS Social Work Supervisor, serves as a Program Specialist /Liaison and trainer for Case Management for Children and Pregnant Women program providers, regional case managers and potential providers. Work includes implementing recruitment, training and monitoring practices for the contracted CPW Providers and Provider agencies.
Provides technical assistance to assigned social work providers as well as regional social workers. Collaborates with HHSC Case Management Division on special assignments. Conducts CPW Pre-planning Sessions with potential providers and follows them through the application process. Works in collaboration with other DSHS SHSS Liaisons from other regions as needed. Adheres to internal and external Case Management policies and procedures. - Case Manager/ Social Worker Provides complex and specialized social work consultation and coordination of services to families of children in the Personal Care Services (PCS) Program and Community First Choice (CFC) programs.
