Overview

Reporting to the Director of Operations, Mobile Case Management will provide street outreach and case management with chronically unhoused clients, requiring knowledge of experience in Housing Forward, mental health, addictive disorders, and benefits eligibility, especially as it relates to the unhoused. This position involves locating and engaging the unhoused on the streets and in encampments, as well as moving unhoused neighbors from the streets, diverting them to available housing, and connecting them with vital resources, while maintaining a relationship with the unhoused clients.

Outreach and engagement includes conducting initial screenings, entering data into HMIS, assisting in the process of getting unhoused on the housing priority list, and providing on-going support. Case Managers will assist clients in communication with and support from established agencies, non-profits, and government programs.

Roles and Responsibilities
  • Engage the unhoused on the streets, encampments, and other public access areas in the DFW Metroplex
  • Conduct intake and assessments outside of the facility setting
  • Provide intensive mobile case management services to participants using a solution first model
  • Assist clients in obtaining required critical documents
  • Assist clients with documentation for housing (CAS)
  • Provide crisis management and intervention
  • Consistent follow-up with clients
  • Verify client eligibility for program services
  • Utilize evidence-based practices, such as motivational interviewing and trauma informed care
  • Work closely with other Case Managers, the Operations Director, and other Metro Relief staff
  • Connect unhoused clients with treatment for substance abuse and mental health services, as well as other resources that are needed, both within the COC provider network and outside those boundaries
  • Help resolve issues and develop solutions for continued success
  • Maintain current documentation and records of encounters, assessments, and referrals, as well as other statistical data required by the program and HMIS
  • Maintain familiarity with community resources for unhoused clients and procurement of new resources
  • Attend weekly staff meetings, team meetings, supervision, and training
  • Other duties that may be assigned by the Operations Director
Qualifications and Education
  • Education:

    Bachelor’s degree in social work, or related field strongly preferred
  • Experience:

    One(1) year experience in case management working with the unhoused population/outreach experience preferred
Required
  • Must be proficient in Microsoft Word, Excel, and the Google Workspace
  • Must have a valid driver’s license and reliable transportation
  • Must be willing to work directly with chronically unhoused individuals with disabilities (physical, mental, substance abuse) who may or may not have a background in criminal activity
  • Musthave at least one year of sobriety
