- Provide direct patient care through office visits, telehealth, and occasional same‑day appointments.
- Perform comprehensive assessments, order and interpret labs/imaging, and diagnose acute and chronic conditions.
- Develop personalized treatment plans that may include lifestyle interventions, supplements, hormone therapies, and prescriptions.
- Educate and empower patients on preventive health and functional medicine principles.
- Collaborate with internal health coaches, nutritionists, mental health providers, and clinical support staff.
- Document visits in the EHR (Next Gen) in a timely and compliant manner.
- Participate in team meetings, quality improvement initiatives, and training sessions.
- Support clinic operations by contributing to a positive, team‑oriented environment.
- Active Oregon license as a Nurse Practitioner, PA‑C, MD, or DO (or eligibility to obtain).
- Board certification in Family Medicine, Internal Medicine, or applicable specialty.
- Strong interest in preventive, functional, and lifestyle medicine.
- Excellent communication and patient relationship skills.
- Comfortable working in a fast‑paced, collaborative environment.
- Proficient in using electronic health records (Next Gen experience is a plus).
- Accepted Nurse Practitioner Licenses: FNP – Family Nurse Practitioner; AGNP – Adult Gerontological Nurse Practitioner; AGNP‑PC – Adult Gerontological Nurse Practitioner.
