ABOUT THE CLUB HEALTH & REHAB CENTER- Located in The Villages, FL, we are a skilled nursing and rehabilitation facility that is dedicated to helping our residents maximize their potential and live their lives to the fullest. The Social Service Director oversees the planning, implementation, and evaluation of social service programs within a healthcare facility (such as a hospital, nursing home, or rehabilitation center). This role ensures that patients’ psychosocial, emotional, and practical needs are met in alignment with medical treatment goals and regulatory standards. The director also supervises social work staff and collaborates with interdisciplinary teams to support high-quality patient care.
KeyResponsibilities
Program Leadership
Develop, implement, and evaluate social service policies and procedures.
Ensure compliance with federal, state, and local healthcare regulations.
Coordinate psychosocial assessments and discharge planning for patients.
Patient & Family Support
Advocate for patients’ rights and dignity.
Provide counseling, crisis intervention, and resource referrals to patients and families.
Address issues such as adjustment to illness, grief, financial hardship, and community reintegration.
Collaboration & Communication
Serve as a liaison between patients, families, medical teams, and community agencies.
Participate in interdisciplinary care conferences and contribute to care planning.
Build partnerships with external agencies for patient support services.
Team Management
Recruit, train, supervise, and evaluate social work staff.
Provide mentorship, professional development, and performance feedback.
Ensure adequate staffing coverage to meet patient needs.
Administrative Duties
Maintain accurate documentation in compliance with HIPAA and facility standards.
Manage departmental budget and resource allocation.
Prepare reports for senior leadership and regulatory agencies as required.
Experience: Minimum 3–5 years of social work experience in a healthcare setting; leadership/supervisory experience preferred.
Skills:
Strong knowledge of healthcare regulations, discharge planning, and case management.
Excellent interpersonal, organizational, and crisis management skills.
Ability to lead teams, resolve conflicts, and foster collaborative working relationships.
