RN Case Manager – Hospice (The Villages, FL)

Location:

The Villages, FL. Requires daily travel to assigned area.

Schedule:

Monday-Friday, 8:00 AM – 5:00 PM with an on‑call weekday rotation.

Compassionate Care Hospice, an Amedisys Company, seeks a dedicated RN Case Manager to provide compassionate, collaborative care to hospice patients and their families.

Onboarding for an RN typically lasts 7‑9 weeks and is individualized according to your needs and experience.

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develop, prepare, and maintain individualized patient care progress records with accuracy and timeliness, according to care center policies.
• Record pain/symptom management changes and outcomes as appropriate.
• Teach patients, families, and caregivers appropriate patient care techniques.
• Regularly communicate with the clinical manager and care team.
• Perform on‑call responsibilities and provide services to patients and families as assigned.
• Other duties as assigned.

• Current RN license in the state(s) where you will work.
• Minimum 1 year of clinical RN experience in an acute healthcare setting.
• Current CPR certification.

• Hospice or home health experience.
• Strong communication and interpersonal skills.
• Ability to work independently and manage multiple priorities.
• Comfortable with daily travel between facilities.
• Proficient in electronic medical record (EMR) systems.
