Requirements:

• Current state license as a Registered Nurse
• Homecare Homebase Software Experience
• OASIS Experience
• Proof of current CPR certification
• Current driver's license
• Must have reliable transportation and insurance
• 1-2 years of experience as a Registered Nurse in a clinical care setting
• 2+ years of Home health experience
• CHPN certification (preferred)

Roles & Responsibilities:

• Assess and respond to the needs of patients and families in home health and hospice settings.
• Develop, document, and evaluate individualized Plans of Care (POC) to ensure quality and continuity of care.
• Educate patients and their families regarding care, medications, safety, and self-care practices.
• Supervise and provide clinical direction to home health aides to ensure quality service delivery.
• Monitor patient pain and implement appropriate pain management strategies.
• Communicate effectively with patients, families, and the interdisciplinary team.
