Chapters Health System in Florida seeks a Clinical Team Leader (CTL) to coordinate daily clinical care for hospice patients within the IDG. You will provide and manage direct patient care, supervise staff, and ensure care aligns with the plan of care.
Qualifications include an active RN license in our state, at least 2 years of nursing experience, EMR/EHR experience, and BLS certification. A valid driver's license and reliable transportation are required to visit patients in home settings.
