This position is based in The Villages, FL serving Lake County

As a Home Health RN, you'll bring exceptional care directly to patients in their homes, helping them maintain their independence and heal safely.

• Conducting in-home
  OASIS assessments (required), evaluations, and developing personalized, goal-driven treatment plans.
• Providing clinical oversight and guidance to Licensed Practical Nurses (LPNs).
• Collaborating with patients, caregivers, and a supportive clinical team to deliver exceptional care.
• Utilizing dictation services to streamline documentation and maximize time for patient care.
• Acute & chronic disease management
• Medication management
• Post-surgical rehabilitation support
• Pain management
• Wound care

• Unencumbered Florida Registered Nurse license (required)
• Home health care experience with OASIS documentation proficiency (required)

• Every-Other-Weekend (Friday-Sunday)(Per Visit Rate)
  No nights or 12-hour shifts required!
