TX Recruiting is seeking an experienced Hospice RN for a growing hospice organization in The Woodlands, TX. This is an excellent opportunity for a compassionate registered nurse who is passionate about providing exceptional end-of-life care to patients and their families.

This role is ideal for a hospice or home health RN who wants to make a meaningful difference in patients’ lives while managing a caseload with both autonomy and support. You will provide skilled nursing care, develop individualized care plans, and work closely with an interdisciplinary team to ensure patients and families receive the highest level of comfort and support.

Responsibilities
  • Manage a caseload of hospice patients and provide skilled nursing care in the home, facility, or other care settings.
  • Complete comprehensive assessments and develop individualized plans of care.
  • Provide symptom and pain management in accordance with each patient’s plan of care.
  • Educate patients, families, and caregivers on the plan of care, disease process, and what to expect in end-of-life care.
  • Coordinate care with physicians and the interdisciplinary team, including social workers, chaplains, aides, and volunteers.
  • Document patient visits and care accurately and in a timely manner.
  • Ensure compliance with hospice policies, clinical standards, and regulatory requirements.
  • Participate in the on-call rotation as needed.
Qualifications
  • Current Texas RN license.
  • Hospice or home health experience strongly preferred.
  • Strong knowledge of hospice patient care, symptom management, and end-of-life care.
  • Excellent clinical assessment, communication, and documentation skills.
  • Compassionate, self-motivated, and comfortable managing a caseload independently.
  • Valid driver’s license and reliable transportation.
  • Organized, dependable, and comfortable working in a fast-paced environment.
