Licensed Practical Nurse

Altru Rehabilitation Hospital is seeking a compassionate Licensed Practical Nurse (LPN) to join our Healthcare & Medical Services team. In this role, you'll provide direct patient care, administer medications, monitor progress, and support rehab treatment plans for inpatient and outpatient populations. Responsibilities

• Provide direct, hands-on nursing care to rehabilitation patients under the direction of RNs and physicians
• Administer medications and treatments safely and accurately according to provider orders and facility policies
• Monitor, record, and report vital signs, patient status, and responses to treatments in the electronic health record
• Support patients with activities of daily living, mobility, and rehabilitation exercises to promote independence and recovery
• Participate in admission, transfer, and discharge processes, including patient assessments and documentation
• Collaborate with the interdisciplinary care team to implement and update individualized care plans
• Reinforce patient and family education regarding conditions, medications, therapies, and post-discharge care instructions
• Maintain strict adherence to infection control, safety, and quality standards within the rehabilitation hospital
• Recognize and promptly report changes in patient condition, escalating concerns to the RN or provider as appropriate
