Position: CLINICAL PHARMACIST

Clinical Pharmacist - Pharmacy Oncology Drugs Description

Pharmacists fill orders for medications, monitor patient medication therapies, provide information, and supervise/direct support personnel. In the absence of the pharmacy manager, a pharmacist may be required to assume the essential responsibilities and duties of the manager. A pharmacist must have the ability to work independently with minimal direct supervision; work cooperatively with health-system and pharmacy staff; handle frequent interruptions and adapt to changes in workload and work schedule;

set priorities, make critical decisions, and respond quickly to emergency requests; exercise sound professional judgement; communicate effectively (orally and in writing); and meet the pharmaceutical careneeds of the patient populations served by the organization (neonatal, pediatric, adolescent, adult, geriatric). Pharmacists must practice in accordance with all applicable legal, regulatory, and accreditation standards.

Repetitive use of hands and fingers (e.g. preparing IV admixtures, use of a computer keyboard); may require lifting and carrying light loads (up to 40 lbs), including boxes, equipment, and IV solutions, and stooping or kneeling; sitting, walking, or standing for long periods of time (4-12 hours) is often necessary; must be able to physically operate the equipment used for the job.

Occupational hazards include potential for exposure to infectious patients or materials and hazardous and toxic substances (including chemotherapy, cytotoxic drugs, and cleaning solutions); potential for sticks or cuts by needles and other sharp items; potential for musculoskeletal injuries if proper lifting and carrying techniques are not used.

Prepares and dispenses medication orders per prescriber requests according to established policies, procedures, and protocols.

Ensures safe, appropriate, and cost-effective medication therapies for patients according to established policies, procedures, and protocols. Reviews all medication orders for the following: patient allergies or potential sensitivities; existing or potential interactions between the medication ordered and food and medications the patient is currently taking; the appropriateness of the medication dose, frequency, and route of administration; current or potential impacts as indicated by laboratory values;

therapeutic duplications; regulatory compliance; and other considerations. After the medication order has been reviewed, all concerns, issues, or questions are clarified with the individual prescriber before dispensing. (MM.05.01.01)

Contributes to high quality and effective operation of the pharmacy departments and the organization. Participates in the quality improvement and medication-use review activities of the department. Ensures ongoing accuracy of inventory quantities and expiration dating. Maintains competence required for current job title/position. Assumes responsibility for own professional education and development and assists in providing education to patients, students, residents, and other healthcare professionals.

Clinical Pharmacist - Cancer Center

Continuously maintains and increases competency in the practice of oncology and outpatient infusion pharmacy. Performs all responsibilities necessary to ensure and promote effective and judicious use of clinical and financial resources of patients and the organization.

Experience / Qualifications

Graduate of an ACPE-accredited School of Pharmacy with a BS Pharmacy or Doctor of Pharmacy degree. Licensed pharmacist or eligible for pharmacist licensure in the State of Georgia.
