TGSC worksites in Thomasville or Cairo

Schedule:

Up to 40 hours/week – scheduled within a 7 ‑day work period

Salary Range: $28,571.29 – $36,023.24 – $62,000.00

The LPN 3 plays a vital role in fulfilling this mission and vision by leading and supporting CRA (Community Residential Alternative) setting, CLS (Community Living Support), and Day Services by providing a person-centered approach to nursing care for people with intellectual and developmental disabilities. The LPN 3 ensures full compliance with applicable local, state, and federal regulations, including those from DBHDD, DCH, and CMS.

This position is essential in maintaining a safe and respectful environment that reflects each individual’s choices, goals, and preferences, while maintaining the highest standards for confidentiality.

Job Summary:

Under supervision of Registered Nurse (RN), provides general direct nursing care to adults with intellectual and developmental disabilities. You will provide prescribed medical treatment and personal care services to individuals in a CRA (Community Residential Alternative) setting, CLS (Community Living Support) and Day Services. Your responsibilities will include providing a person-centered nursing approach, observing and reporting health-related data, coordinating medical care, documenting nursing care, scheduling medical appointments, providing transportation to and from medical appointments and conducting health-related training for unlicensed personnel.

You will be responsible for performing quality assurance checks and advocating for the individuals supported and helping to develop community supports ensuring the department provides services in a non-judgmental, non-discriminatory manner that considers cultural diversity and age appropriateness so that autonomy, rights and dignity are preserved. This is an integral role in ensuring the individuals receive the best possible care and support to thrive.

Provide a person-centered approach to nursing care to people with intellectual and developmental disabilities

Administer prescribed medications and treatments per physician orders

Perform nursing procedures (e.g. wound care, tube feeding, catheter care etc.) as required
