All employees have a role in performance improvement and are expected to interact collaboratively with co-workers and other contacts to provide consistent, high-quality, youth focused services.
Program DescriptionMARCH is an acronym that stands for Mobilizing Access to Resource Care and Hope and is a program that is an expansion of Intensive Customized Care Coordination (IC3) services. This adds a pre‑enrollment care coordination stage to IC3 by
1) facilitating referrals and linkage to services and supports for youth not meeting IC3 eligibility criteria and
2) increasing enrollment for youth meeting IC3 eligibility criteria. MARCH also offers additional elements such as continuous family and youth engagement, System of Care philosophy, System of Care elements training and education, suicide prevention, emphasis on diverse underserved populations, collaborative partnerships, and trauma and grief informed care.
The care coordinator provides a broad range of case management services to patients/clients/youths and their families. Conducts patient/client/youth intakes, participates in treatment team planning, and performs crisis intervention. Assist in dealing with personal and social problems. May provide behavioral and emotional support to youths and families and/or serve as a liaison for social services. May perform case management duties. Assist individuals in identifying and gaining access to required services and supports, as well as medical, social, educational, development and other services and supports.
