Per Visit Physical Therapist

Job Summary:

Under the supervision of the Home Care Rehabilitation Supervisor, the Per Visit Physical Therapist organizes and administers prescribed physical therapy programs in an effort to reduce and/or prevent disability and to assist patients in achieving maximum functional potential.

Per Visit Physical Therapist

Job Responsibilities:

  • Evaluates and treats patients as prescribed by referring physician. Records treatment, response, progress and discharge information in patient's electronic chart using portable electronic device. Educates patient, family and significant others regarding condition, treatment and home program. Routinely confers with physician and other practitioners in person, by phone, or in writing, regarding progress, regression or change of plan.
  • Provides clinical supervision of the Physical Therapist Assistant related to the implementation of the patient's plan of care, in accordance with Medicare Conditions of Participation and state practice regulations. This supervision is specific to patient care and does not include administrative or employment oversight
  • Orients, instructs, and directs other Health Care Team members as needed.

Education and Experience Requirements:
  • B.S. Degree and graduate from an accredited school of physical therapy required.
  • Connecticut license or Connecticut license eligible required.
  • CPR Certification required.
  • Minimum of one year experience preferred.
  • Valid Connecticut Driver's License and proof of current insurance coverage required.

Per Visit Physical Therapist Knowledge

Skills and Abilities:

  • Must be proficient in Reasoning, Math, Reading, Writing, and Speaking:
    Speak with Poise, voice control and confidence using correct English and well-modulated voice.
  • Data:
    Determines time, place, and sequence of operations or actions to be taken on the basis of analysis of data, executing determination of and/or reporting on events.
  • People:
    Follows instructions and procedures in fulfilling duties. May attend to the needs, requests of people. Immediate response is usually involved.
  • Things:
    Uses body members, hand tools, special devices to work, move, carry objects or materials. Involves little latitude for judgement with regard to attainment of standards.
  • Uses common sense and special medical skills to care for the sick or handicapped.
  • Understands technical information from supervisors, charts, reference books, manuals and labels.
  • Ability to use Portable Electronic Device to record/maintain patient information pertaining to Plan of Care.
  • Uses eyes, hands and fingers with skill.
  • Works fast and accurately in an emergency.
  • Communicates with people when they are sick, handicapped or nervous.
  • Changes from one duty to another frequently.
  • Identifies and acts on priorities for smooth departmental operations.
  • Maintains discretion in handling confidential nature of information available.
  • Keeps up to date on trends and developments in the field of physical medicine and health
