Intake Coordinator RN

Job Summary:
The Intake Coordinator is responsible for screening, processing, coordinating all clinical referrals and services. Responsible for obtaining and maintaining updated information about the department, community and State programs.
Key Responsibilities:
- Responsible for coordinating the admission process for new and returning home health and hospice patients.
- Determines eligibility for home health or hospice services based on initial referral information to include the patient's medical needs, insurance reimbursement, and homebound status.
- Determines the most appropriate clinical team for the patient to receive evaluation and care services from based on clinical presentation, caregiver status, equipment needs, and available agency resources.
- Monitors patient status throughout the health care continuum, assisting with the patient's transitional care needs as necessary. Completes transfer OASIS as needed, communicating with inpatient facilities and care managers to ensure safe transitions of care.
- Determines if a Face-to-Face Encounter has been performed, collaborating with the patient's provider to schedule a Face-to-Face Encounter within regulatory guidelines.
- Triages referrals and schedules them in collaboration with clinical managers.
- Ensures verification of MD licenses of Physicians not on staff at DKH by phone or via internet screens maintained by each State's web site.
- Appropriately delegates processing of referral information to the Intake Assistant and Clinical Liaison to ensure patients are admitted to DKHC services timely.
- Tracks and reports referrals that are not taken onto service to ordering providers and referral sources.
- Determines initial referral ICD-10 diagnoses in collaboration with home care ICD-10 coder to ensure covered home health and hospice diagnoses to support an accurate HHRG and RAP submission.
Education and Experience Requirements:
- Current license to perform as a registered nurse in the State of Connecticut required.
- Valid Connecticut Driver's License and proof of current insurance coverage required.
- Graduate from an accredited School of Nursing required.
- Two years of recent home health and/or case management experience as a registered nurse.
- Previous experience in acute care or critical care experience as a registered nurse or an equivalent level of experience.
Knowledge
Skills and Abilities:
- Excellent working knowledge of state and federal home health and hospice regulations.
- Excellent computer skills and ability to quickly learn various software and internet-based applications.
- Ability to drive an automobile year round in varying weather conditions.
- Ability to follow verbal and written instructions, assess priorities, record information accurately.
- Demonstrates sincere concern in caring for patients who are ill or handicapped and willing to be accountable for maintaining Day Kimball Hospital standards on confidentiality.
- Ability to assist with heavy lifting and capable of working under pressure. Ability to use arms, eyes, hands and fingers with skill.
- Ability to communicate with and relate to referral sources, providers, patients and their families with patience and tact.
- Maintains an awareness of patient's age and developmental level when identifying how best to conduct patient care.
