Job Description
1. Referral Management
Process and review referrals from public agencies and partners.
Ensure accurate and timely data entry into DSS SSNET.
Liaise with referrers to clarify information and follow up on case status.
2. TriagingAssess and prioritise cases based on urgency and risk level.
Conduct initial screenings and identify high‑risk cases.
Allocate resources according to needs and service capacity.
3. Communication & ReportingMaintain communication with vulnerable individuals, SSOs, and community partners.
Prepare reports on referral trends and case outcomes.
Participate in team meetings and case discussions.
4. Event Planning & CoordinationOrganise engagement sessions, forums, and appreciation events.
Support coordination of learning forums and management sessions.
5. Quality ImprovementIdentify and implement process improvements.
Support quality initiatives and ensure compliance with guidelines.
6. Administrative SupportCoordinate outreach arrangements with SSOs and MSF HQ.
Update relevant databases and support PATH Fund administration.
Assist in initiatives such as Street Count.
Specific Requirements1. Qualifications
Tertiary qualification in any discipline.
Experience in social service or case management is an advantage.
2. Skills & CompetenciesStrong written and verbal communication skills.
Good analytical and critical thinking abilities.
Strong interpersonal skills and ability to work effectively in a team.
3. Personal AttributesCommitted, self‑driven, and proactive.
Dynamic, resilient, and persevering.
Willing to work beyond regular office hours when required.
